SUPPORTED INDEPENDENT LIVING

LIVE YOUR WAY WITH THE RIGHT SUPPORT

Supported Independent Living (SIL) helps you live as independently as possible, while building your skills. Whether you need support with personal care tasks, cooking meals or cleaning your house, Youth Connections prides itself in delivering supports tailored to your needs.

WHO IS ELIGIBLE FOR SIL?

This type of living support is suited to young people aged 18 and over with high support needs, meaning you need a significant amount of help though your day in your home. SIL supports are assessed based on how much support you need to live as independently as possible.

ACCESS SIL FUNDING

If you are 18 or older and want to find out if SIL funding is appropriate to you, it is important to consider the following:

  • What are your individual circumstances and needs?

  • What are your independent living goals and preferences?

  • What kind of independence do you want, including living skills and capacity-building needs

  • Do you need to make modifications to your home or acquire assistive technology to be more independent

  • What information, reports and assessments do I need to provide and gather from health professionals and providers

  • What significant person-to-person supports do I need to meet my living needs

  • Is this the best living support suited to me

  • Will this give me the best value for money
