NDIS Support Coordination
Skool’s Out is an after-school and school holiday support program for high school aged young people living with disABILITY.
All activities are designed to build social skills, life skills and engage participants in age-appropriate, recreation & development.
All programs are flexible and allow young people to make contributions to their programs.
Skool’s Out strives to break down barriers and support young people to develop their confidence and independence in a fun and relaxed environment.
Our staff don’t believe in sitting by the sidelines. They’re passionate, active and engage our participants as friends and equals.
Transcendence is a gaming hub created by Youth Connections in partnership with FundAbility.
Participants are supported to develop social connection and have fun gaming in a safe and supported environment.
Gaming nights take place regularly on Fridays for junior and senior groups of young people.
Our Next Step Program (NSP) and Transition to Work (TTW) are post school programs for young people with a disability.
NSP and TTW offer a safe learning environment where participants can attend up to four days per week. Our post school programs support participants with improving independence through building on life and work skills. NSP and TTW are designed to assist our participants in gaining skills that will assist them in finding and keeping a job.
Our post school programs are a supportive network with many benefits, our participants can choose to participate in our volunteer work program, complete nationally recognised training courses. Participants also have access to case management that can provide individual employment support to complete resumes, interview preparation and potential employment. We offer participant focused sessions such as goal setting, steps to achieve these goals, self-reflection, confidence building, teamwork and life skills such as budgeting, cooking, health, fitness and community participation.
The days of attendance vary depending on your location, your NDIS package, your individual choices and needs.
Youth Connections staff are able to assist participants one on one within the community to accomplish their NDIS goals.
Our service caters to the individual needs of our participants and strives to help our participants reach their goals. Our individual support is flexible to be delivered any day of the week with a minimum of 2 hours to a maximum of 10.
Youth Connections can also cater for small groups within the community. Our community engagement can be catered to allow for participants to engage with one or two friends/fellow participants depending on the individual needs of the participants.
The Out & About(O&A) program is an inclusive program that engages young people with local activities.
Content is picked by the participants and includes group based activities such as visual arts, media, fitness, life skills development, recreation and volunteer work. We frequently visit local attractions such as galleries, festivals, community events, beaches and national parks. We offer a fun and engaging learning environment both community and centre based, our vision is to empower participant’s and encourage them to focus on their ABILITIES whilst finding their place with-in our community. Out & About is run four days per week across our Central Coast and Hunter locations.
